Return the form to Bob Farrell at PO Box 3134, Oswego, New York 13126 or via e-mail at nysasa@twcny.rr.com. All JO requests will be screened by Bev Valachovic, JO Commissioner. Deadline for requests is Monday, October 1, 2007.
